Hey Doc, I Can't Put My Finger On It, But.......

Doesn't it make you feel good when you are respeceted enough by the docs to just be able to say "hey, I'm not exactly sure what's going on here, but this is what I see, and I'm worried", and they say yeah, that doesn't sound right.........

Had a pt come in through the ER during the night, primary made rounds in the AM, but she just didn't look right to me, just too SOB with minimal activity, the doc thought it was just a bronchial infection superimposed on bad lungs....after nagging him later he ordered ABG's and chest CT...sure enough it was a bilateral PE.

We get dinged for the "failure to rescue"'s, but nothing is ever said for the "good catch"....that's why we're here, right?

I get more of a warm fuzzy when the doc tells me "good job" than a pt or family.

I feel like the good catch happens a lot but we don't get much credit for it.
My good catch tonight....
I called a doc about a newly admitted dehydrated (among other things) pt, when I looked at the labs, I noticed the K was 2.9. There was nothing ordered PO/IV to replace it, not even in her maintenance fluids (K level wasn't even mentioned in Md progress note). I asked the Dr why this was and she said "Oh, I guess we overlooked that" and she then ordered a host of K replacements.

nice catch!!!it does feel totally good when you catch something and let the
MD know, especially when the patient has been in the ER for hours/days.
nice job! not only are us ER RNs doing ER nursing, but also floor nursing, another thing to add to our resumes!!!

I like what VegRN said: There are many,"good catches", we just don't get credit for them. Good job ! When I'm precepting someone that is just what I try to reinforce. We need to be proactive in care of pt's. Our hospital has a rapid responce team that seems to be very helpful to nurses. It does just that. When a new nurse or any nurse for that matter feels something isn't right with a pt. they can call the team. We evaluate the pt. and work with the pt's RN to come up with a solution. We don't take over, we just aid. It seems to work really well with new nurses because it gives us a chance to do a lot of teaching. Even if they just have a question about labs or EKG readings, we're there to help. It's main goal however, is to improve pt. outcomes. So far it seems to have worked well. So , I guess you were your own rapid response team.....good job.
