To CM/ UR: Do you use InterQual criteria for all Medicare admissions?

I am wondering how other hospitals around the country apply InterQual , M&R or some other criteria to approve or deny Medicare admissions - AND - if an admission does not meet criteria what is the mechanism for stopping the bill from being dropped to CMS?
