Hi,

I need some help! I am a community matron from Sussex, England who is a case manager for patients with long term conditions. I am coming to Philadelphia the last 2 weeks of March 2007 and would like to shadow a similar case manager there for a day, does anyone know how i could contact someone to facilitate this?
