AOA accreditation/patient problem list/careplans

During recent survey with AOA it was indicated our faculty needs to develop a "patient problem list" that is part of the patient record. This is in addition to the Care plan. Of course the list would be used to develop the care plan. Anyone have an example of a problem list form ?
