I work in LTC, for many years. The A&I reports are getting CRAZY long! What are yours like, is it just us? We are required to get a statement from EVERY staff member who worked on that unit for the previous 24 hours, regardless if it happened right in front of us. There are reams of forms to fill out.
Just wondering if our facility is paranoid!

We have a Post Fall Assessment Log which is just like it implies. For 3 days, we do ortho's et document if the intervention was effective, ect. Then we have a Fall Reassessment, and Accident/Incident form just in case their are injuries. Overall, I am pretty satisfied with the paperwork. When our new DON came in she combined many forms for us so that we were not doing the same thing over et over. And we only document if there was a witness to and incident et who it was. Then they can follow up as necessary with the witness.

I work in LTC too, the paperwork is ridiculous!! LTC always get a bad rap on why nurses can't spend enough time with residents, it's because we're doing so much paperwork!!

Yeah and while you are at the desk filling out 20 pages of paper work related to the fall another patient falls. Why? Because the nurse was at the desk instead of at the bedside. Bedside nursing is not only getting ridiculous it is becoming impossible.
