I have a question about the online application. What do I put on the "Professional Title Obtained"? This is located under the Schools Attended (college specifically). I put Registered Nurse but I was told by somebody that I should have put "BS Nursing" because I did not obtain my RN from simply graduating with a BSN as I had to take the local licensure exam to obtain the RN. Should I write to CGFNS to change it, or is that understood and accepted that after graduating, I obtained the RN by passing the local board? Also, I need to add information on the "Name of diploma" blanks for high school and college - I need to put "High School Diploma" and "BS Nursing diploma" on these blanks. The reason I didn't was because I thought it was understood that since "Secondary Education" is written there and bec. I'm applying for VisaScreen (nurse), I didn't have to. But now it's bothering me that I didn't fill these up.
I cannot edit the online application as the attestation has already been signed and the fee already been paid for.

P.S. "BSN" is a degree, and "RN" is a title so I put RN instead of BSN. Was I wrong in doing this and do I need to write to CGFNS to change it?

You cannot make any changes to what you have submitted so it does not matter at this point.
But for others that are reading this, this is the reason that I never advise anyone to use the on-line application; many have problems with a wrong date and then things get delayed for months trying to correct it.
Always better to print out the application, enter the information, then review it, and then send it in. Doing it on-line does not save you any time at all, and there are no visas at this time. So no hurry in actually mailing the application in. But it can cost you extra months when trying to correct something that you submitted and attested to being correct in the first place.

It will not make any difference in this case, there is nothing to write them about.

hello suzanne,
may i ask u one thing about visa screen:
1) i ahve two courses post secondary, AB Psychology adn BSNursing.Should i also send my diploma in AB psych?
2) How do i sign the attestation?its confusing
3) the $ 10 fee, should i make a separate draft for it?
4) theer is an item where i need to state that i worked in _________ for a _____years. I have yet to recieve any hosiptal experience. what should i put on it?

Not Suzanne but will answer some of your questions.
In the Philippines, the transcript from the previous courses are already integrated with your nursing transcript for second coursers. Look over the transcript that your nursing school gave you, it is there already. The diploma is not needed. Even your diploma as BSN is not needed. Never has. Only the transcripts are needed, verification of your local license, and a copy of your H.S. diploma if you graduated within 10 years, if more than 10 years - it is waived.
For all other questions being asked that does not pertain to you such as hospital exp., you either leave blank or place N/A. They will not hold it against you. It is only asked there for information or record purposes.
What attestation are you talking about? Is what you're talking about is item 20 on page 29 in the application packet? That is just you attesting that all information you gave are true and complete and if were later found out that there was fraud that they have the right to revoke the visa screen certificate anytime and that you give them authority to disclose any information regarding your file upon your designation of certain organizations or employers to receive such information.
You have to sign it. It is an agreement between you and CGFNS. It is more for legalities.
You can combine fees in a single bank draft. The total fee is $448. Not sure where the extra $10 you are talking about comes from.

1. the bRN charges $ 10 for copy of exam results,should i make a separate draft( aside from the $ 6o that i need to pay for the verification).
2. Can you post a sample of the signature for the vs application pls?

The $10 is to be given to the BRN and the back draft reflecting the name of the BRN. You have to make a separate request that goes directly to the BRN. You coupled it to your VSC application post, so it appeared as if you were still talking about the VSC application process solely.
Anyways, yes you can make a single bank draft and combine the 2 fees together for a total of $70. Just include a letter that states that the $10 is for the copy of test results (with the appropriate add. on where to send it) and the $60 is for the other service.
CGFNS/ICHP likes for their applicants to just print their name using English characters. No use of Chinese/Korean/Japanese/Arabic or any local alphabet characters that they cannot read or not familiar with. With this regard you also cannot use your usual signature if it is the scribbles or what I call ECG-type signatures that only you can decipher.
It's either you just plainly print your whole name (use your whole legal name from your birth cert.) if you want to be sure or if your long-hand writing is very legible you can use that too (just make sure you can replicate this so don't use this if you are not used with long hand writing) Bottom line is that they want something they can read and understand.
*Note that this special instruction for a signature is unique fr CGFNS and ICHP applications forms. Any other forms that do not have special instructions just like you have read from the VSC form and asks for your signature, you always do your usual signature.

If the course wasn't completed then you can't write anything on the degree obtained part. You can write you took it and what dates attended as it is asked in the form then just either leave it blank or put N/A or "not completed course".
As for the transcripts part, that question has been answered many times already and also just answered it again just recently as well.
