Me and my BF have been together for 4 years, and he help me out a lot during my nursing school. We do want to get married, but I do not want to get married to him because of the GC unless it was absolutely the last choice, because I do not want to get married for the wrong reason.
My thing is, if the hospital send out all the document and then I get married with him, is that more trouble and loops that I have to go through, than just wait after get married, I want to work a while and at least pay off all of our student debts before get married.

There are no green cards available for you right now with hospital petitioning, nothing past the I-140 can be submitted. And once your OPT is completed, then you will have to leave the US within 60 days if things do not open up by then, and we do not expect them to for the AOS processing.
It is entirely up to the officials at immigration as to what they will accept from you and that includes proof that there has been a relationship in place for sometime, not just being used to be able to stay in the US. They make the final decision on this.
People get married because that is something that they truly want to do, not as an excuse to be able to remain in the US. You are looking at several years before having a chance at a green card thru an employer, and that can be longer depending on your country of birth.
