Depends on what visa they are looking at, H1C appears to be one we appear to see a lot of and only approx 15 hospitals in the country have this and there are problems ie security, environmental, staff retention and if you loose your job you have x amount of day s to find an employer with the same status to take your visa on otherwise you have to leave. H1B are usually gone in hours and generally taken up by IT or specialists, Generally RN does not meet that requirements

Currently no way to change status once in the US and no idea when the window will open due to the high applications submitted last time but probably looking at a couple of years. I would suggest using the search facility
