One of my favorites I see way too often:

Physical Therapy for progressive ambulation on pts (yes, I see it often) who have been paraplegic or quads for years

May ambulate PRN, this was in a chart of a resident that was w/c bound and could not stand.

A teaching hospital where I worked, one of the foriegn residents wrote "eyes and nose q shift". We all laughed for days over that one.
