charting bloopers

While doing clinicals as a student, I found this error while reviewing a pts chart:

0130 Pt very restless and crying all night, contacted MD who gave VO for a shot that would put her to sleep.

0245 Pt lying on back in bed. Looks very peaceful after being put to sleep.


Im not kidding!!

I WORK IN A NURSING HOME, AND WE GET TO KNOW THE DR'S QUITE WELL, AS WELL AS KNOWING OUR RESIDENTS MEDICATION REGIME, SO IT WAS QUITE A CHUCKLE AFTER A PARTICUARLY HECTIC DAY TO NOTE THE FOLLOWING ORDER FROM OUR NURSE PRACTITIONER WHO IS QUITE REFINED.

THE ORDER READ: "HOLD ASS TIMES 3 DAYS AND EVAL AFTER PT"
SHE MEANT TO "HOLD ASA..."
