Question HIPPA for independent nurses

I am starting my own private nursing co. and am having trouble finding information about HIPPA guidelines and private nursing. Specifically, where do I get a release for the patient to sign so that I can give the MD a copy of my charting? And can I mail these or do I need to deliver them to keep privacy practices in check? So much paperwork!
Can anyone help with links to private independent nurse information, forms, rules, etc.?
