The form asks if I'm taking any medication and what for.

I take Prozac (20mg) for PMDD and migraine prevention. I started out just taking it 14 days a month but discovered that I was almost migraine free during that time so the doctor said I could take it every day.

Since that didn't completely stop the migraines, I also take Maxalt when needed.


Also, it asks if I've ever had a back injury. I have spondylolythesis (lumbar) from childhood trauma that was diagnosed over 10 years ago when I was in 4 car wrecks in 6 months. I haven't had back pain associated with the injury in years. The radiologist that read my x-rays (I worked in radiology and they were done at his suggestion...off the books.) said that I shouldn't have any issues with it, as long as I don't gain too much weight.

Should I include this as a back injury?

Will it affect me getting into a nursing program? I know that a doctor has to sign off on my health but I don't see a doctor for this and haven't since 1997.

Thanks in advance for any info on these two things.

Hi! I would report that you had a previous back injury but that it is no longer a problem and that you have been "fixed."
About the medicine, just list it in the list of medications you currently take. List the prn drug also. You shouldn't have a problem getting into nursing school with this information.

This is simply my opinion but my thoughts on this were if it isn't something that would have affected my ability to complete nursing school it wasn't any of their business.

That's my thought as well...this is the application for the CNA. I don't know if the NS I'm going to will ask for a list of my medications.
