I am having a very difficult time with Care Plans
I have been reading some of the post here
But if ANYONE has any they can share as examples, (maybe something will click) I would be EVER so grateful!!!

Just put all nurses in the subject
so I don't think its junk mail

ANY help would be most appreciated!!!!

The Nurses's pocket Guide: Diagnosis, Interventions, and rationales (9th ed)has been a great resource. You can look up the medical diagnosis in the back and it gives you a complete list of all nursing diagnosis that apply. Good luck!

Can anyone help with collaborative nur dx? I have to do six of these along with six regular nur dx. In fact, I'm not even really sure what a collab. nur dx is. And to make it worse, I have preclinical today and my instructor will probably not be there and they (meaning all twelve) have to be done by clinical tomorrow. Anyone with informative hints, websites, advice, etc? Thanks, I really, REALLY appreciate it.

the collaborative problem may be also known as potential complications within each disease.i.e. atelectasis in a copd pt.

Our school had a "Careplan workshop" Tuesday.I have a sheet of notes she gave us if you will e-mail me your e-mail address I will copy them and send them to you. It should really help! Just remember: Problem as stated by NANDA (Diagnostic Category) R/T Etiology/cause (contributing factors) AEB Signs/Symptoms (Defining Characteristics) example: Impaired skin Integrity R/T Immobility AMB 2 inch diameter ulcer on L hip...... now when you have High risk you will not have a AMB. so it will only have 2 parts. but email me your email address and I will reply to you with the notes my instructor gave us I HOPE THIS HELPS!

I finally got my grade back, which was worth 50% of my grade, and received...............................

100% on my Care Plan!!!!

The way I understand the whole care plan thing is as follows:
Nursing Diagnosis=Acute pain What is your problem? The patient is in pain
The r/t = why is your pt in pain (i.e. abdominal incision, migraines, whatever)
AEB= what is the pt doing to let you know they are in pain (i.e. guarding behaivors, vital sign changes, groaning, or states I am in pain)

We have to do NOC and NIC care plans. Was doing good until they made us go to that format, now on May 12,2006 I think I will have a bon fire in my back yard and NiC and NOC will be on top of the pile.
