Can someone Explain???-CVVH, CVVHDF etc???

What is the difference between CVVH, CVVHDF, and all the others. Are they just techincal difficulties. Also, how long will a patient remain on CVVH, with no improvment of kidney function, until the Doc recommends dialysis.
