Regulations want us to have something in place that shows we monitor for side effects.I am in the process of trying to develop a form that CRMA staff can use as an aid to help them to monitor for side effects of psych meds in the clients we serve. I think the check-off type of format would be the most user friendly and a good tool for me to follow up on clients with. Anyone using sonething like this? I am just looking for some ideas so i can put something together. Thanks!

Not sure if it would help but we have a one page AIMS form we use. On admission (and if Dr. orders after) we fill it out. It list involuntary muscle movements of face, hands mouth etc. You circle numbers from 0 - 4 for a variety of categories and there are two YES/NO questions regarding motor movement. If you think this might help I could try and get a copy - its probably a WORD document is someones office. Let me know - by posting or sending PM. I will check on here within next 2 - 3 days.

The Liverpool University Neuroleptic Side Effect Rating scale (Day et al 1996) sounds like the kind of thing you're after. It's a 51 item checklist of side effects and patients are asked to rate how much they have experienced the side effect over the past month. It includes extrapyramidal side effects, hormonal side effects and so on. As it was originally devised as a research tool, it also includes some "red herrings" to avoid the possibility of people over-rating themselves. I guess you could take these out if you weren't keen on that idea: it does seem a bit underhand. I have a copy you're welcome to have.

It sounds like I could use that form! I'm can reconstruct it for our use, but no sense in reinventing the wheel totally. I'll PM you and send you my email address...
