We normally have 20-25 pts per unit & are staffed with RNs & Psych Techs. We are trying to make shift change as meaningful as possible - to give the most important information in the best way.

I was wondering what fellow nurses were doing for shift report. Is it taped, face to face,......do you use any paper forms to convey patient information from shift to shift, as in a the old Kardex system that you hand write?

Any information for us would be appreciated.
