I have come up with Ineffective Tissue Perfusion r/t pulmonary artery occlusion AEB hypoxemia, cyanosis, and hypotension.

Now that is the best one I have come up with. I cant figure out any other good NSG Dx because they are very broad.

Any help?

I apologize for finding your post so late. I do a search every day for people looking for help with nursing diagnoses and care planning and for some reason your post slipped by me. So sorry.

I can see you're having problems with this. My first question was, do you have a nursing diagnosis book? It would be really helpful is you had a book like Nursing Diagnosis Handbook: A Guide to Planning Care, 7th Edition, by Betty J. Ackley and Gail B. Ladwig since it has an index in it where you can look up your three symptoms and get ideas for possible nursing diagnoses. An alternative is to use their online care plan constructor (http://www1.us.elsevierhealth.com/Evolve/Ackley/NDH6e/Constructor/ - go down to the section "Start a New Care Plan". You want to go into the alphabetic index links for Symptoms, Diagnoses, Procedures, Interventions & Clinical States.), but I must tell you that the site isn't large enough to include links to all 172 nursing diagnoses or to contain all the information in the book, so you won't always get complete information there.

Here are some ideas for potential nursing diagnostic statements that you could use:

• Impaired Gas Exchange R/T alteration in supply of oxygen or the inability to transport oxygen or changes in the alveolar-capillary membranes AEB hypoxemia and cyanosis
• Ineffective Tissue Perfusion: Cardiopulmonary R/T decreased afterload AEB hypoxemia, cyanosis and hypotension
• Decreased Cardiac Output R/T decreased prelaod AEB hypotension
• Ineffective Breathing Pattern R/T secretions in respiratory track AEB [you need to supply assessment data here if you have any]
• Sleep Deprivation R/T inability to breathe AEB [you need to supply assessment data here if you have any]
• Acute Confusion R/T decreased supply of oxygen to the brain AEB [you need to supply assessment data here if you have any]
