Community-based/focused nursing

Hi! I'm looking for a little elaboration regarding community-based and community-focused nursing. Coud someone help me with a comparison and contrast? From my understanding community-based nursing takes place in the community while community-focused nursing can take place anywhere, but it is a view that sees the community as the patient. Any input?
