I work in a surgeon's office where we schedule surgeries to be preformed at the hospital. We schedule a pre-op H & P with the patient's primary physician who then clears the patient, or not, for surgery. There is a pre-op form that the primary physician sends to the admission dept at the hospital along with any labs, X-ray, EKG, stress test that the primary doc may feel necessary to clear the pt for surgery. There are some tests that are required by anesthesia for particular pts, based on age and health conditions.
