Consent for Flu Shot

Can somebody help me with flu shot consent form, Is it a requirement (Law) to witness the consent form? I am trying to develop a policy and procedure for Influenza and Pneumonia vaccines and other shots. I know in bigger facilities It's a must to witness consents, I was told that for vaccines it is not necessary. If you have forms that you can share I will appreciate it

I'm a DON in an assisted living facility, and I made up my own form, which consisted of an explanation of the risks and benefits of the flu vaccine and a place for the staff member to sign, certifying that to their knowledge they were not in their first trimester of pregnancy. and not allergic to eggs or any other component of the vaccine. As long as they know what they're getting into, and as long as I have a signed physician's order allowing me to administer the vaccine, I figured I'm covered.
