Are you looking to actually create a form? Can you be a little more specific as to what you actually need, and I am sure that we can help you. Your system is slightly different where you are...........

Different vocabulary for one thing....
I currently work in a specialty pediatric hospital and we have the parents fill out the history form, then we go over it with them. Our form is actually about five pages long.

Are you asking about filling out an actual health physical form? Nurses over here are not responsible for doing that unless it is a pediatric nurse practitioner. We still do complete assessments each shift.

Is this for a clinic or a hospital peds form? Difference in that also.
