Question Fall and Pressure wound risk assessments

Is anyone currently using a tool to do fall assessments with children? It is a JCAHO requirement, yet I can't find a pediatric tool, and am wondering if this is just an adult requirement (the JCAHO site doesn't specify). Also wondering if anyone is using a skin risk assessment tool. We heard this is a 2007 JCAHO standard, and wondering if any peds units use one and how often. Thanks!!
