medication review

We have a large population of elderly in our congregation. I see that it would be a value to them to help them put together a list of their medications, medical history, allergies, etc. I would also like to be able to take a look for interactions, if possible.

Has anyone done a program like this? Anyone have any good forms/programs?

Any info appreciated.

HI susi_q,

Have you ever heard of the Vial of Life program? We have worked with it through our first responder/fire units here in town. You would have to call around to see if anyone is using it, they would probably love to have more help with it. Call senior resource people, fire/rescue. Here is a good site to give you info on it, plus some downloadable, printable decals.

They use a baggie on the site, we use a medicine bottle. One of the drug stores donated some for our use. I'd be happy to help you with any info I can give you...just ask.

WOW! I have never heard of them before, but am really impressed with what I see. That is exactly the information I was looking to include - and what a great system. I think I'll get a couple of my colleagues together and see if we can put something together to start using the program. Thanks so much for sharing.

One of our local assisted living facilities uses the vial of life...It is a wonderful thing...It is kept in a baggie inside of the fridge...makes life a lot easier when someone comes in unresponsive and can't answer questions...The staff at the assisted living facility update them once a month. So they are almost always up to date...
