As far as I know individual anesthesia departments set their own criteria. I think they get some guidance from ASA, but still each anesthesiologist have their own interpretation. We just put down on paper what they are at our hospital. I would get them for you ,but I am on vacation for 3 weeks. Check with the ASA web site. Good luck, Mike

PS, they change yearly so you can't say they are set in concrete
