It is our institution's policy to NOT use a "clot buster" in a PAC because you can't be sure you can withdraw 100% of the clot buster back out. If it had been me, I would have reaccessed the pt and flushed it according to protocol (saline & heparin). Unfortunate for the patient, I know, but it could clot off before they go back to the MD in a week. It has been my experience that PAC clot off easily.....Hope it works out well. Don't beat yourself up, just learn from it.
