Outpatient primary nursing?

I was wondering how many outpatient centers do "primary nursing". I'm thinking of instituting it in our unit for patient continuity. Sadly, depending on what's going on with some nurses on some days some things can slip through the cracks and I'd like more follow up care. It seems like it's the same ones starting the line, or taking care of the one with a frequent occluded line, and in chart review I'm finding that Doxil is never started by X nurse, Rituxan is avoided by X nurse, X nurse never charts discharge status...etc.
Do you do primary or are there no assignments in your outpatient center?
