Chemotherapy during hemodyalysis

I was wondering if any oncology nurses could answer this question for me. Is it common for an ESRD patient on HD to get chemotherapy, specifically CHOP, for non-Hodgkins Tcell lymphoma? How does the body deal with the chemo if the kidneys have failed? Any insight would be gretly appreciated.

Common? Not in my experience. However, there are a number of trials currently looking at using CHOP or R-CHOP in ESRD patients. There was one Hopkins trial where they did not find these patients to be at a significantly greater risk for complications. But you would think that TLS would be a much bigger problem. On the other hand, we have had to take patients to HD due to their TLS. Sorry, I can't be more help as I am relatively new to oncology.

We had a young guy who was constantly teetering on the brink of renal failure. We had him dialyzed 1 hour post-chemo and were very particular with his pre and post hydration. It sounds like something similar would have to be done for your pt. w/renal failure.

depends...if he is ESRF why chemo...usually not a chemo candidate.

if his renal issues are related to his lymphoma and tx then possibly the hope is that tx will improve his overall and renal condition.

it also depends on his age, comorbidities and likelihood of benefit of chemo vs risk
