Hello everbody,
I am doing a nursing care plan for pregnant woman and I can't think of any nursing diagnosis.

Patient is 36 year old Gravida 1 para 0 @ 33 weeks gestation.She takes prenatal vitamins, no other medications. she denies pain today but over the last few days she has had times of intermittent dull, low bacck pain 3/10. Her blood pressure today is 106/64 and her prepregnant BP was 100/60. There is no signs of edema. Pt urine dipstick is negative for protein. Pt weight is 154, prepregnant weight was 130. Pt states that the baby has been quiet toady. Fundal height is 33cm
Fetal non stess is reactive FHR is 150.
The nursing diagnosis I come up with is
Deficient knowledge of fetal movements r/t lack of information about problems associated with decrased fetal movements manifested by the statement that the baby has been quiet today.
I don't what else. I am thinking about something about her age?? or blood pressure but these seems normal to me. Because Pt is 33 weeks gestation and i think she already have her triple screening and amniocentesis in second trimester. Please guys help me out!!!!!!!!!!!!!!!!!!!!!

Risk for pain r/t uterine contractions
Readiness for enhanced knowledge r/t developmental processes of labor.

I'm not a nurse yet, and I haven't started nursing school. But I'm a mom, and I know ANY intermitant low dull back pain is a BIG sign to watch for! They are a sign of preterm/term labor, because the pain can be caused by contractions...

Knowledge deficit r/t pregnancy, r/t upcoming labor, r/t infant care, r/t fetal kick counts, etc.
body image concern, can't remember the exact nanda diagnosis, r/t growing belly and normal changes of pregnancy.

Otherwise sounds like a normal pregnancy without any major concerns.

risk anxiety R/T upcoming labor and delivery, gravida 1 para 0
interrupted family processes R/T birth of baby

maybe you could do something with nutrition... how's her H&H? maybe do a diet recall for a couple days- bet you could find something there.

She had a non stress test so we know she isn't in Pre term labor

Altered comfort (anyone who has been pregnant or the SO of a pregnant woman knows that)
Altered sleep/rest patterns (ditto)
Knowledge deficit (this is always a huge one for primiparas)
Altered nutritional requirements (this also applies postpartum, esp. if breastfeeding)
Potential alteration in sexual activity patterns (I've long since forgotten the exact wording of the pattern)
Anxiety

Amazing what you can find for what we would call a fairly normal healthy pregnancy!!

All the care plans I needed for OB nursing could be found in my OB textbook. Don't they include these anymore?

Ours has a selection of some along with some case studies. I tend not to use them though, because I have found that the actual diagnostic statement is incorrect in a lot of cases (using a r/t that is a medical diagnosis which is a no-no!). They can give you some good general ideas though.
