I know this does not answer the OPs questions, but the best lactation consultant I ever had was the housekeeper at the hospital I delivered baby #3 in. he was the 1st child I had attempted to breast feed and I just could not get the hang of it, I had met with the lactation consultant and the nurses tried to help me, but it was hurting so bad and i was so stressed out, here comes the lady to take out my trash and i have tears in my eyes and she asks how I am doing and I tell her, "Oh OK but I just cant seem to get this nursing thing down". she comes over and says "Oh girl your doing it all wrong" she gathers some pillows, props the baby up, pulls over a trash can to prop my feet on, positions the baby just so and gave me great tips. My only regret is that I never sent her a thank you card!!!
I had already decided I was going to quit till I met her, and I am happy to say I nursed him for almost a year, and then next baby I nursed with ease.
Hats off to housekeeping!!!
