Title of Study: Barriers to Implementation of Electronic Documentation Among Nurse Practitioners



You are invited to participate in this survey regarding electronic documentation in healthcare. I am a graduate student at the University of Connecticut, and am conducting this survey as part of my course work. I am interested in finding out attitudes and opinions among nurse practitioners that are barriers to the utilization of electronic documentation.

Your participation in this study will require completion of an online survey. This should take approximately fifteen minutes of your time. Your participation will be anonymous and you will not be contacted again in the future. You will not be paid for being in this study. We believe this survey does not involve any risk to you. Although you may find it interesting to participate in this study, there will be no direct benefit to you from your participation.
