Weaning Oxgyen

what is your expert opinion and research-based evidence on weaning neonates from oxgyen theapy, espeically those oxgyen dependent neonates/infants?

Does your unit has a guideline and/or protocol on it?

In my unit, we often do by trial and error when they're on .01 - .02 L/min, however, they often depend this small amount of oxgyen. Oxgyen is also initited by MD. We also put these baby back on oxgyen when they are having bottle or being breast fed as they often don't torelate well and have problem with sucking, swallowing and breathing.

No standing protocol. We start at 1L then will wean them to .5L. and yes, even wean down to .25L which is like the baby is sitting in a breeze and I think is stupid.
If the can't tolerate a fed without O2 they generally stay on it continuous.
We never have a baby on 2L either, but we'll have them on CPAP with a PEEP of 5 21% o2!Something else I think is stupid!
Oh now you got me strted! LOL!!

We have kids we wean to <0.1/min and just can't get them off that. One option the docs order sometimes is what we call "blended O2". They start at 0.1L/min of 100% oxygen like everybody else, then we wean the FIO2 until (hopefully) we reach 21% and then we should be able to take off the cannula then which is only blowing a tiny amount of room air. In truth, I've never seen a kid placed on blended that weaned easily off of it. Maybe after several weeks and then who says they wouldn't have weaned off anyway off the traditional NC? I remember constantly fiddling with one of our chronics (former 23 weeker) to get her blended down to 0.1L at 30% FIO2. She was on that for several weeks but wouldn't wean any further but needed to go home. Can't send kids home on blended so she goes home on a NC at 1/8 L/min. The only thing I think it's really great for is kids with cyanotic heart defects that need the absolutely lowest possible amount of oxygen.
