We use NaAcetate (Full, 1/2 and 1/4) or NS (Full, 1/2 or 1/4). Both with 0.5 units of heparin/mL. We usually run at 1 mL/hr, sometimes 0.5 mL/hr on the smaller babies. We change fluids based on lab results daily (or sometimes more). Umbilical lines are placed with betadine prep. We do use Chloraprep for PICCs, Broviac dressing changes and art sticks.

We use a mix with Vaminolac and water in all our arteriall lines, umbilical or peripherial. Heparin is never used in my unit.

Umbilical lines are cleaned wih NS for a baby under 30 weeks, for all others itīs chlorhexadine.

Itīan amino acid mixture (with all 18 essential and non-essenseal amoni acids required for protein synteses), where the propotions tries to mimic the content of breastmilk (includes taurin as well).

Itīs a way of giving the baby more protein and to use all iv fluids for nutrition.

Works just like amino acids in regular food but since itīs given directly into the systemcirkulation it gets to skip absorbtion through the bowel and bypasses the liver.

Interesting......

Have you guys had any issues with arteriospasm?

Where I used to work all our arterial fluids were 500cc 1/2NS with 250u heparin and 60mg Papaverine @1cc/hr to prevent arteriospasm......

We use chloraprep for all insertions, UAC and PIV. We actually had started by using the Sali-wipes post chloraprep but saw more burns that way. THe thought was that the saline or sterile water dripped the chlor. over more surface area and that contributed to the burns...<shrugs> I have noticed that now that they don't wipe we haven't seen the burns...they were ugly too.


Our most common uac fluid is 0.5 ml/hr Sodium Acetate /c hep.

Not sure on prep solution but we run our standard D10 TPN with lipids through ours. (at my old job I should say) our lipids are not seperate from the TPN and of course it is heparinized.

Edit: of course they may change to a D5, D12.5 or whatever based on the kids bld sugars.
