trach+ spit fistula/esophagostomy?

Hey, has anyone out there run into any babies with both? We have a baby with a spit fistula for an unrepaired EA, and he also has tracheomalacia and really needs a trach. But, no one has dealt with both of those here, so we want a plan of how to deal with both, prior to putting a second hole in his neck!!

Any experiences would be most appreciated.

Hmmmm. I've never had a kid with both. No help from me, unfortunately....
