PPHN treatment of ELBW infants

Just curious as to was other facilities are using to treat severe PPHN in these ELBW infants. It seems we have been running a special lately.

So what standard and controversial treatments do you use?

I don't recall having an ELBW pt with PPHN. Lots of term kids, mec asp and such. Most end up on the oscillator, some get nitric, rarely go out for ECMO.

I've not seen one, or if they've had it, it was called something else.

our PPHNers are much closer to term, then we, like the above poster, use iNO, HFOV, and ECMO.

We don't see it too frequently, but we treat the little ones the same way that we treat the larger ones: HFOV, iNO, sedation. The only difference is that ECMO isn't an option d/t their size.

bamagirl- have you guys found anything else that works on the little ones?

We don't typically see PPHN in the littlest ones, but our standard is not unlike the above posters, oscillator-nitric-sedation. Recently did treat a 30 weeker for it, but thats the youngest one I've had.
