Role of "Primary Nurse" in independent private duty nursing

Has anyone ever come across a description of the responsibilities of a Primary Nurse on an independent home care case? I've worked as a Home Care Provider for the past 13 years and have learned the role of Primary Nurse, or Case Manager, through experience. I am currently serving this role on a Medicaid case, and the other nurses have decided that I have "overstepped my bounds" by "telling them what to do" when we all have the same level of education. I fill the majority of the weekday daytime hours and have had most of the contact with non-nursing members of the medical team., and have simply been the liason. I have never worked on a case that didn't have a leader who assumes responsibility for achieving continuity of care and communications related to changes in the care plan, yet they clain they have never seen this. Please help!!
