We try to them out as soon as possible, usually it's around 24-30 hours for a routine heart. If a patient is only one 1 pressor/inotrope (i.e. say levo or dobutamine), swan will usually get pulled around 4am the next morning. Swan will stay in if the patient remains on multiple pressors/inotropes for a couple days.

About 24 hrs unless things are not progressing as planned then longer.

Most hospitals (have travelled & worked in 15 hospitals to date) the swan is out before the patient leaves the OR. It is really an unnecessary risk that gives you little to no information for a simple CABG. It's useful for monitoring PA pressure after a valve or transplant but other than that it doesn't really help you much. You can determine if your CO/CI is ok for your patient by monitoring vitals, LOC< and UOP so it's not really needed.

We keep the swan for 24-48 hours depending on what gtts they're on and how well they are progressing and we are a major cardiac surgery facility with above average case loads. If you pull the swan in the OR, it was basically pointless to put it in in the first place.
