Ramsay Sedation Score

Hi everybody... i'm a silent reader all this while.. but now.. i need help from those who have more experience..

My hospital... i mean.. the hospital that I'm working for.. doesn't have a format for ramsay score.. I know score 1-6 meant for what... but for us to create the form format for clinical use (as if dr's specific sedation rate according to the score) for nurses reference is not available.

Beside an information on sedation score, specific range rate/strength of sedation.. what is the other vital information that is required in the form if we need to create one?
