Wink ICU assessment flow sheets

I'm trying to update and revamp our old ICU assessment flow sheet. It is inadequate and doesn't include all the key elements for a thorough assessment. Any ideas, websites, links or examples people are willing to share!

Hmm, no one responded? We had a lecture on documentation, and I would like to see a few examples of flow sheets if anyone has resources or links?
