Are any of you willing to share your "brains" to ADD nurse?

Hello,

I am finishing up with school and just read the thread about ADD nurses getting any disability leeway...anyways, I noticed a bunch of you said you made your own report sheets, etc... are you willing to share those with me or anyone else reading this thread that could use it? I am interested in seeing how ADD nurses organize their days...and I need a starting point towards making my own sheet or set of brains for working the floor.
