nursing coordinator supportive living

I have taken a job as a nursing coordinator for a supportive living facility. Wanted to know if anyone has a template for the old-fashion kardex that was used many years ago. I need to find a way for my nurses to use their resident service plan and incorporate the care plan in their shift report. I thought by using the kardex they would have all the information for their residents at hand. If any one has any ideas how to incorprate the care plan and a 24 hour report for daily use i would love to hear about them.

Also can anyone define the difference in the nurse practice act 2008 LPN from RN

Any help appreciated TRACYR
