A significant change is indicated when the patient has a decline or improvement in two areas that is not temporary and is unexpected. For instance....
You have a patient that walked and transfered independantly, needed only minimal assist with adls, was continent and ate good. He or she is now non-ambulatory, incontinent and is loosing weight......This is a significant change. Another expample, The patient develops stage 2 or better wounds and has had a 5-10% weight loss.
One occurance that always requires a significant change is new G-tube whether or not the patient's over all self-sufficiency is the same or not.
