Hi to MDS coord

I have a question about New rug group (Extensive plus rehab)
Section P for IV medication
when new admitted came from acute hosp. when we do 1st 5 day assessment . do you count IV med from acute hosp ? and how far you look backto get higher rug group and to get high pay on PPS .if you don't have transfer ordered for IV continue to SNF for skill

Hi - I'm afraid I don't know the answer, as we do assessments on the 14th day, plus Canadian MDS seems to be a bit different that American.

I know that I have an email address that I can send quesitons to and they respond back very quickly. Usually 1 -2 days. I am sure you must have something like that - I would suggest emailing them. I would think that if the pt was still on IV meds you should be able to count that somehow.

IV medications have a 14 day look back period and you can count this if your 14 day window extends back into the acute care stay (which it often does for a 5 day MDS). I always, always make sure I have a copy of every MAR from the hospital that will cover the 14 day look back period - I then highlight the IV med and attach the sheet to the rest of my MDS documentation.

Example:

10/1/06 - IV Vanco while in acute care
10/10/06 - Resident admitted to the nursing home
10/14/06 - End of 5 day observation
Count the IV on this assessment.


Next example:
10/9/06 - IV Vanco while in acute care
10/10/06 - Resident admitted to LTC
10/14/06 - End of 5 day observation - capture IV med
10/22/06 - (day 13) end of 14 day observation - capture IV med again cuz you are still in 14 day window to capture that.

You then get to keep the higher RUG score for full 30 days instead of just 14 days. This makes a huge financial impact on the facilities reimbursement.
