5 day assessment

I have a question about a resident admitted to our SNF Med A with therapy. His 5 day is day eight . He is now being discharged and transfered to another facility closer to his home. Discharge date is day 13. Do I need to do the entire RAPS and all or a 5 day and then D/C MDS? Thanks for your help

All you need is the 5-day assm't. The Adm assm't (w/ RAPs) is only required when the resident is still in facility by midnight of day 14.

However, if you initially coded the assm't as 5-day/Adm (combined), you have two choices:
1. Change it to 5-day alone. Submit. Code DC = 8.
2. If you choose to keep it combined, follow the instructions in RAI, p3-239. Code DC=6

I will forgo #2. Just make it a 5-day and be done with it.

I would just do the 5 day and not waste time doing RAPs. They are only required if the resident is still there at midnight on day 14. Also you need to do a "discharge prior to initial" assessment when the resident leaves.
