Infusion Ctr flow sheet

Hello all,
I am starting at our new infusion center next week and have oriented there for a few days already. They have asked me to "revamp" the flow sheet that is currently in use. (honestly it sucks!) We want to keep it to one page if possible. I am looking for any and all ideas and if you have one you are willing to share...thanks!

Our infusion center is about 90% chemo, so our flow sheet is pretty much specific to that. We assess vitals, appetite, weight, advance directive, and pain.
