49? A little low for anything but glucose. BUT, maybe this person is usually low. Depends on the person.
What has happened in the past? Do they normally run low? Ask 'em.

With 49,you should be treating for hypoglycemia instead of giving hyperglycemics no matter if it is short,intermediate or long acting.You should hold it!!!
