diabetic teaching plan help, help

I need a specific diabetic teaching plan for a new onset type 2 DM, this plan needs to include objectives, assessment, resources, timelines etc. I have looked at various websites but I am having trouble putting it all together for my professor. It needs to be specific and in a chart.
If anyone has any ideas or tools please help
