go to Tribal areas (area where you live), click onto your area and ask for recommendations for your area. Check out the Homebirth subforum.
Also at that same site, there is a subforum for birth professionals, ask there too.
You should be able to get any and all questions answered at the mothering.com forums. They will know, too, about the ss# and birth certificate issues.
You could also try the forums at Midwiferytoday.com looking for a midwife.
