Hello,
I'm new to this site and I hope that I'm posting in the proper place?
My grandmother is hospice and I'm hoping that I'll be able to get some help here. There is so many family members coming in and out and everything is getting very confusing.
We are looking for a charting system to use so that showers can be logged, meds given, meal prep. and so forth. The hospice we are using uses a computer program so I guess i have to make my own paper charting system. Any ideas as to what I put on it? Or where I can see an "example" chart ?

I guess you could set it up like they set up the med charts in long term care. They use an 8 1/2 X 11 size paper with the days of the month running across the long side of the paper. Down the left hand side are listed each med and the times it is to be given. When the meds are given, the box is initialed. For prn meds the time and date are written in as it is given. The same thing can be done with meals served and accepted, showers given, etc.

Like, what cares go on there ???
So much is happening that everytime I try and create something, I over look 1 more thing and its driving me insane. I wanna make sure I'm covered in every aspect when I present my final draft!!

No, I don't think there is any "standard" chart. I think each place tends to create their own and there is one for food acceptance, another for recording bowel movements, another for meds given, another for treatments done, another for care given, yada yada. This is mainly because the state requires such documentation to prove that things are actually being done. I would think in a home situation you probably don't want to get that detailed because it will become too confusing and complicated for people to use. I'm assuming that you mainly want this so that the various people involved in care will know what the others have done and what they need to do yet at a glance. I would give some thought to what is TRULY important information and stick to that.

Question is this for your own knowledge or for legal documentation? If for your own knowledge, perhaps a dry erase board to communicate would be helpful.
