Documentation of narcotics incident in the home

My home care hospice caregiver and patient's relative (though not POA) claims to have accidentally dropped a bunch of vicodin down the drain. That, together with other nonspecific indicators raised some eyebrows. As any prudent nurse would do I increased the number of skilled nursing visits for med monitoring. As expected, she was not happy and refused to give me access to all the narcs for a count. I documented everything on my visit note and called my office for further instructions. Of course she threatened to sue. It was an ugly day.

I'm wondering how I should document all of the conversations I had with my superiors as an internal matter, just to cover my own butt as there will be consequences.

I've just done an admit that smells like there's likely to become a narc diversion problem. I document everything that occurs with the Pt. and CG. I don't document conversations with IDT/supervisor unless there's action not on the POC.

Simply document what you have informed supervisors about.
This can get ugly, but you know you are just trying to do your job. These family members who are addicts are really difficult.
