Discipline Frequencies

Does anyone have information on how to write the frequencies for the various disciplines such as HHA, SN, MSW, etc ? We usually put a range so we don't have to write orders for increased or decreased visits. Thanks for the help. Love this site.

We use ranges too but we try to keep them meaningful....such as "1-3 and prn" or "3-5 and prn" for HHA. The "and prn" covers your oncall/emergency visits. The only team member you are allowed to have just prn is the Medical Director.

In the home setting we do the same as above, usually 2-3x/wk and prn for the nurses. We're usually pretty specific on the # of days for CNAs. In the facility, we have to be very specific down to which days of the week and AM or PM for Nsg, CNA. SW and Chaplain can be a little less specific.

Thanks for the input. We have always done ranges in case of bad weather( ha,ha ) but my PCC just wanted to check with others.

This is how we write the orders:
SN: 1-5x week and 3-4prn for sx management x13weeks
MSW: 1-4xmo and 3-4 prn for distress
Chaplain: same
HHA: 2h 5x week x13 wks
Volunteer: 1-5xmo and 3 prn

We weren't allowed to have a range of more than one, such as 2-3x wk and PRN. This was for all discplines and if you went over that, it was an unscheduled POC to cover yourself..... HTH

The company I work for has us do ranges of:
SN 2-4x month with 48 prn
HHA 1-3x week with 32 prn
MSW 1-4x month with 48 prn

I think they are strange ranges. The only thing it helps me with is our computer charting. Full assesments in the computer are required q2 weeks. Not that full assessments aren't done every visit but the PRN visit on the computer takes much less time. I've never had a hospice pt that only needed 2-4 visits a month.

I've been using ranges for SN (1-2/wk and prn) and HHA (2-3/wk and prn) but I've been notified that if you use a range that you must visit the patient the upper range the first week of admit. I would appreciate your input. Thanks!
